EVALUATION PROCESS AND FEE SCHEDULE  

Completion of rating inventories: Parents/caregivers and teachers will be asked to complete a series of rating inventories prior to the evaluation session to assist in assessing a child’s emotional, behavioral and social functioning across settings.

Intake Interview: An intake interview is conducted with parents to discuss the presenting concerns in detail and obtain information regarding your child’s medical, academic, psychological and social history. This typically takes 1 – 1.5 hours. With parent consent, Dr. Dorfman subsequently speaks with your child’s teacher(s) and other relevant providers involved in his/her care (e.g., tutor, psychologist) to gather further background information.

Testing: Testing takes several hours and may be broken up over more than one session. The testing schedule and length of the evaluation depends on several factors including an individual’s age, reason for testing, and/or general behavior and stamina during the evaluation.

Feedback Session: A feedback session is scheduled within 1 - 2 weeks of completing testing. During this 1.5 – 2 hour session, Dr. Dorfman describes the results of the evaluation in detail and provides appropriate educational and treatment recommendations. Dr. Dorfman helps to connect families with appropriate local providers for any recommended interventions and supportive services.

Report: A written report documenting test findings, diagnostic impressions and recommendations is provided following the feedback session. Dr. Dorfman generates comprehensive reports that are straightforward and easy to read for parents and other potential audiences (e.g., educators, therapists).

Additional Services: Services that are not part of the standard evaluation are billed at an hourly rate and include classroom observation, participation in impartial hearings, and consultation with attorneys.

Fees: Dr. Dorfman does not directly accept insurance and is considered an out-of-network provider.  It is recommended that you contact your insurance company prior to scheduling an appointment to determine your out-of-network coverage for neuropsychological testing (CPT codes 96116, 96132, 96133, 96136 & 96137) and therapy services (CPT codes 90791, 90832, 90834, 90846 & 90847). Dr. Dorfman provides a detailed receipt for services rendered that includes all appropriate codes that can be submitted for out-of-network reimbursement.
